I think the system has to have been filled and bled properly for the surge tank to function as intended. A leak somewhere would also cause improper recovery of levels.

If your radiator cap does not seal correctly it will not maintain vacuum to suck the extra coolant back in from the reservoir. There's a section in the manual on how to test it and what acceptable pressures it should have. Other than that if your line is clogged or has a kink that would restrict the flow. You correct it is a very simple system, as long as everything is working ok
