Well if the bomb drops, we are probably all screwed anyhow. But I think it will be an economic catastropy first, then global warming will slowly kill crops and food sources.
Then it will be free for all.

you only have to have enough radiation shielding to and supplies to last a few days untill the extremely short half live of post blast particals subsides.

don't expect it to be like churnoble, that wasn't a bomb.

underground fort with 3' of dirt is only needed to sheild from radiation (or zombies )
